360 ASSESSMENT
The purpose
of this questionnaire and report is to increase leadership effectiveness through
self – awareness. This will lead to enhancing the understanding of personal
strengths as well as possible areas for development with regards to Leadership.
We prefer to facilitate the content of the report in a feedback session. This
also ensures that information gained is validated and owned by the leader.

treated with the utmost confidentiality and should be stored in a secure
location.

Readers of the report
should not place undue significance on any of these results, realizing that the
instruments used in this report are not infallible or without restriction.

Please note: Psychometric
tests and behavioural assessments should never be used as the sole criterion
for making selection, promotion or any other personnel related decisions.

In light of the reality
that people are likely to develop, the contents of this report cannot be assumed
to be meaningful or accurate after a period of approximately 18 months.

Thanks for helping
us collecting information on the person you are evaluating. He/
she will be attending the ground-breaking course on Leadership and in
preparation we would like to know more about his/her leadership capabilities.
The questionnaire is designed to identify existing strengths and development
areas. The results will be shared with the person at the workshop and we will
be able to focus on specific development areas. Your answers will be treated
confidentially.
On the next few pages are 45
statements describing ideal leadership behaviours. Please read each
statement carefully and evaluate the person in terms of frequency he/she
engages in the behaviour described, using a rating scale, where:
1 = Almost Never
2 = Seldom
3 = Sometimes
4 = Usually
5 = Almost Always
